1、參加本項目磋商的潛在供應商必須具備《政府采購法》第二十二條規定的資格條件；

2、參加本項目磋商的潛在供應商須在黑龍江省 注冊登記并經審核合格；

3、供應商若為生產企業，必須具有所投產品醫療器械生產許可證或第一類醫療器械生產備案憑證；若為代理企業，必須具有第二類、三類醫療器械經營許可證或第二類醫療器械經營備案憑證，并提供所投產品醫療器械生產許可證或第一類醫療器械生產備案憑證。投標產品屬于醫療器械的，必須具有醫療器械注冊證或第一類醫療器械備案憑證；不作為醫療器械管理的，需提供國食藥監械頒發的不作為醫療器械管理的文件，且以上證照在投標有效期內均為有效；

4、單位負責人為同一人或者存在直接控股、管理關系的不同供應商，不得參加同一合同項下的政府采購活動。除單一來源采購項目外，為采購項目提供整體設計、規范編制或者項目管理、監理、檢測等服務的供應商，不得再參加該采購項目的其他采購活動。

5、擬參加本項目的潛在供應商、法定代表人參加政府采購活動近三年內，在經營活動中沒有重大違法記錄，在“信用中國”網站( )、中國執行信息公開網( /）、中國 ( )等渠道列入失信被執行人、重大稅收違法案件當事人名單、政府采購嚴重違法失信行為記錄名單及其他不符合相關法律規定條件的供應商，不得參與本次磋商采購活動。
